Capitol Police Records Request

To obtain a copy of a Police Report, click on Records Request and print the form (it is in .pdf format, you will need Adobe Acrobat). After completing the form, fax it to the Capitol Police Records Department at (602) 364-0026. You can then pick up and sign for the records at Capitol Police Headquarters at 1700 W. Washington, Phoenix on the Basement level, RoomB-15.
